Qantas A380 Flew for Two Days With Mechanic's Work Light Left Inside Wing

A Qantas Airbus A380 flew two international passenger flights with a mechanic's work light left inside its left wing, according to a newly released report from the Australian Transport Safety Bureau (ATSB).

 

The light went undetected through both flights before Qantas's own tool-tracking system flagged it as unreturned, prompting a search that recovered it. Investigators found that the light had been left behind after maintenance work on the aircraft's air conditioning system at Sydney Airport, then remained lodged in the wing structure as the aircraft returned to service.

What Happened

 

The incident involved a Qantas A380, registered VH-OQK and around 15 years old, one of the largest passenger aircraft in the airline's fleet. According to the ATSB, a four-person maintenance team, made up of one licensed aircraft maintenance engineer and three additional engineers, used a 33-centimeter Makita LED work light while replacing a sensor on the aircraft's air generation unit on January 7, 2026. Two of the engineers placed the light inside an access panel on the wing's inner leading edge to illuminate the confined, hot workspace, switched it off, and closed the panel without retrieving it.

 

The aircraft was then cleared for service and flew from Sydney to Dallas Fort Worth and back, a round trip of roughly two days covering more than 27,000 kilometers, before the light was found and removed on January 9. Had both flights operated at full capacity, they could have carried up to 970 passengers combined.

 

The ATSB confirmed the aircraft landed safely on both legs of the journey. No passengers or crew were injured, and Qantas said the light had no impact on the aircraft's operation.

 

How the Tool Was Discovered

 

Rather than being found during a routine inspection, the light's absence came to light after staff reviewed an "unreturned tool report" and noticed it had been signed out against the aircraft two days earlier without being logged as returned. The engineer who had checked it out could not confirm it had been brought back and raised the possibility it remained inside the wing, prompting a search that located it.

 

Qantas voluntarily reported the incident to the ATSB, which then opened an investigation into how the light was missed.

 

Findings From the Investigation

 

The ATSB found that three separate safety checks failed to catch the missing light: a foreign object clearance inspection conducted at the end of the maintenance task, a check when tools were returned, and an unreturned-tool check at the end of the shift. 

 

Sources describing the report do not indicate the light was ever missing from a formal tool inventory; rather, it had been properly signed out and tracked, but its return was never verified across those three checkpoints.

 

The ATSB emphasized that while the aircraft was not placed in immediate danger on this occasion, foreign objects left inside aircraft structures can pose a significant risk to safe operation, potentially interfering with control cables, fuel lines, or wiring if they shift during flight.

Qantas Response

 

Qantas, whose own engineers and on-airport tool facilities were involved rather than an external maintenance contractor, has since introduced a mandatory pre-clearance check to confirm all tooling is accounted for after maintenance and formed a dedicated tooling working group to explore further improvements, including new tracking technologies.

 

This was the second such incident involving a Qantas A380 in recent years. In December 2023, a 1.25-meter nylon turning tool was left inside an A380 engine following maintenance in Los Angeles and remained undetected for 34 flights, about 294 hours, before being found in January 2024. 

 

 

What Comes Next

 

The ATSB has published its findings and safety messaging, and Qantas has accepted them, continuing to roll out changes to its tool control procedures. No regulatory action against the airline has been announced; Australia's Civil Aviation Safety Authority would typically review such findings as part of its ongoing oversight of maintenance operations.
